It is not a surprise if he has a JT, AT, 88-99. It is a bit surprising if he has an overpair, which he decides to slowplay, but the most surprising would be a set. He slowplayed it expecting us to bluff or go for value on the R.

I would say 53 or 89 but it all depends on his table image which is impossible to guess from one hand, if he had three of a kind on the flop why check?, if he had 53 you could have 89, you were surprised so he had 89!

BB has 3h2h I guess. That are cards that can be known as very suprised. Villain had almost a straight also. And finally hero had 2 hearts, so nearly impossible to guess should be that villain had also 2 hearts.
Id say fish had 5♠:Q♠: because since fish mindset he could get a flushdraw, in the Flop he just called to see next card, Turn opens a OESD so he called again, and luckily River brought him glory
