The "McDonald's/fast food only for a month" experiment has been replicated a bunch of times, including some scientific studies under lab conditions, and no one suffered nearly as much as Morgan Spurlock from the Super Size Me movie.
The best example would be a study at Linköping University in Sweden, which had even harsher conditions than the ones in the movie, requiring subjects to take in even more calories per day (over 6000, Spurlock averaged around 5000) and also prohibiting physical exercise. Yeah, some people pounced on the opportunity to participate...
Results were extremely varied, with some people having to drop out after reaching the maximum allowed weight gain of 15% before the end of the study, while others coped quite well all the way through and only gained a few pounds. But no one suffered any of the psychological ailments or the serious medical risks (liver problems etc.) like Spurlock in the movie.
Other people who did milder "McDonald's only" experiments, who didn't force themselves to overeat and stuck to reasonable portions, actually reported losing weight during that time. There are a bunch of those.
Theories for Spurlock's ailments include previously undiagnosed liver problems, a too sudden transition from his vegetarian diet to fast food only*, side effects of steroids, or simply dramatic exaggeration for the sake of the movie. I don't care, but I call BS. McDonald's is pretty much the easiest target in the world anyway.
*If I remember correctly, this guy puked during his first McDonald's meal in the movie. Couldn't he have realized at that point that this wasn't going to end well, and that he's probably not the most representative person for all of this?
