you should defenitely be more aggresive in my opinion but dont show the outcome it can influence. so i would say raise to 0.4 before the flop. and than u did expect him to make an contibet or what ? you cannot see youself behind against most pocketpairs and broadways are beaten for sure. so u can easily raise and start thinking if he calls when he re-raises i would guess he hit trips or an overpair but all other overpairs beat you so than you can easily fold. but when u like slowplaying than i think it was an acceptable way, but getting three bets at every step is rather bad. SO dont slowplay hands that aren t monster is my conclusion.

Big leak preflop, 2 limpers and you raise to 3BB.. make it 6BB here.
it's very simple.. you open raise 4BB.. and if someone limps you make it 4BB + 1BB per limper.

On the flop, standard c-bet make it about 3/4 pot.. like $0.75
Why you check there ?
Turn & river same story.

Like ichmagmich says, you are way to passive..
Be a lot more aggresive..

Questions:

Why you raise to 3BB if there already 2 limpers in the pot ?
And, why you check the flop ?

it is actually painful to look back at this hand as it played out

Well when u checked all street u showed weakness so he kept barelling and once in a while donks like this hit their draws.

Badly played pre and postflop. This is partly why these suckouts happen. Note his aggressive play too.

Preflop:
Make it 0.6$ to go 4BB+2BB one for each limper.

Postflop:

cbet the flop , bet the turn bet the river. We bet for value in those those streets.

If you are going to slowplay this hand, you shouldnt call river since not many people bluff 3 streets in a row so big. Also, board is paired(trips and FH possible, flush got there and 45 got there.
