on flop ck/call w FD & gutshot is OK IMO
on turn with 12 outs it is still OK to call IMO
on river push is the best option, right ? He will never fold 2 pairs or set on NL10$ I guess

Preflop: looks fine

Postflop: You have the nuts on the river and since that card is essentially a blank you won't risk him checking back too many strong hands. So check/push is fine.
