You might be seeing card removal effects.

In the example you give, the K:h is one of the heart outs for Q:h2:h, so some of the equity shifts to the K:hK:c hand.

Dead cards.
The only time I ever used the dead card box is when I called an shortie's all-in in a tournament w/ suited broadways.

Another player in later position 3Bet and I called.
The original shover had A:s9:h I put those cards in the "dead" box so I could see what the equities were in the side pot. The player that re-raised had A:d5:c

The board was 8:s4:s9:d
If I didn't put the A:s and 9:h in the dead box, it distorted the results:

This is what it looked like with the cards in the position boxes:

Not a big difference, but a difference

Note, I just made this hand up -- I don't remember them all that exactly!
The general tone of the hand is pretty close though.
