A lot of the time, I am facing the situation where I am UTG or OOP, and my hand is good enough for a 3bet. So I do the 3bet as standard play, the issue is when I am not making hands OTF or OTT. So I think it is something that exploitable. How do you play such scenarios?
You decide mostly based on 'Fold To Flop CBet' of villain?
Do you flat pre hoping to catch with les money involved?
Do you CB OTF pushing away the opponents? sizing?
Do you X/F if you completely miss OTF?
How do you decide and based on which reasons here?

This spots, are spots that I am bleeding money. I mean I am not losing a big pot, but something like 25, 30 BB in general. It's OK when once or twice, but if it is happening alot, I end up losing 4 times a 25BB which is a stack of 100BB over the course of the session.
Any recommendation to stop bleeding here?
