SB is an unknown

Preflop: I think the mistake stems from 4b'ing as a bluff preflop. Since villain is an unknown I have no reason to believe that he would be 3b'ing me particularly wide OOP. I think it's best to assume I am up against something like JJ+, AQ+.

as played...
I'm not quite sure what to put villain on when he flats the 4b OOP but I would expect the vast majority of players would do this with QQ+ (sometimes even AA only because they're scared of overcards to their hand so they would just normally 5b all-in, having said that we are 200bb effective)

Flop: When villain checks to me should I be inclined to stab or take a free card? I think that this mostly depends on how often we expect him to call 4b's OOP with some A-hi hands. In any case I felt that his range would mostly consist of PP's that were planning on x/raising the flop.

OTT: Once he donks here I think he always has JJ+. I clearly don't have the correct odds (immediate or implied) to continue.

If we bet here we will most likely isolate ourselves against pocket pairs and A high flush draws. I think this is well played.

The 4b is ok. I would wait to collect some more info about his 3b/ fold to 4b. But still you should have a default range vs unknown too.

I wouldn't 4bet this hand without info that he 3bets alot (in general or vs Hero), but it's still totally fine imo. I think we should cbet this flop. We have a FD and two overs + alot of fold equity vs better hands like AX. This flop doesn't hit his range at all.

This hand I think is too low to 3bet in this situation. Further it's well played IMO
